UK "CardioVascular Disease rIsk ConTrol in Survivorship" Research Study Participants Needed

Researchers at the University of Kentucky would like to share an opportunity with you to participate in the CardioVascular disease rIsk ConTrol in Survivorship (CVICS) research study with the goals of:

1. Assessing multilevel (provider and organizational) needs, barriers and expectations regarding the implementation of cardio-oncology services, and
2. Engaging stakeholders to develop a novel multilevel intervention to implement C-O services and deliver relevant care.

Below you will find a link to a survey that asks for your opinions and perspectives regarding cardio-oncology services. The survey will take 20 minutes to complete and is confidential. Your feedback will be used to inform delivery of cardio-oncology of services.
